The Nurse Outreach Consultant contributes to the goals of the Medicare Advantage Quality/HEDIS programs through implementation of member engagement tactics (outbound calls, digital media, mail campaigns, etc.), and actively participates in quality and process improvement activities.

This position is in a hybrid workforce classification with required work on cite at our Mountlake Terrace campus in WA state with additional... flex to work from home 2 days a week.

  • *What you will do:**
  • Serves as a project manager and consultant for member engagement strategy implementation, making sure individual campaigns or projects consistently deliver against established clinical, business, and corporate objectives.

  • Engage members and utilize a culturally sensitive approach that addresses barriers to clinical HEDIS measures and promotes improved health outcomes, including performing medication reconciliation post-discharge.

  • Document a plan for achieving member engagement goals, objectives, and deliverables in partnership with business leaders and support functions.

  • Perform a wide variety of technical tasks such as drafting materials, promotional activities, coordinating program with internal and external resources, facilitating utilization of external resources, etc.

  • Provide progress reports to stakeholders and modify plan due to changes in priorities

  • Capture regular reporting of metrics associated with performance related to member engagement work. Track outcome measures, evaluation tools, and protocols for member engagement program.

  • Work with other areas of the company to improve member engagement activities using observed concerns, member complaints, and provider feedback as the foundation for quality improvement.

  • Facilitate communication and coordination among the health care team to minimize fragmentation, eliminate duplication and maximize delivery of appropriate care.

  • Provide interventions focused on removing the member's barriers to health improvement; promote positive behavior change using motivational interviewing techniques.

  • *What you will bring:**
  • Registered Nurse plus 4 additional years of relevant experience (Required)

  • Active WA state RN license (Required)

  • Master's Degree in population health, healthcare administration, or other relevant field (Preferred)

  • 4 years of experience in the health insurance or healthcare field, including work with member/customer engagement (Preferred)

  • Demonstrated experience in quantitative analysis, HEDIS Quality, and/or project management.

  • SNF Nurse experience (Preferred)

  • Acute or Ambulatory Care Nurse experience (Preferred)
